Patient Info: Your First Visit

Before your first visit, please be prepared to complete all insurance and health information forms that will allow us to begin your dental treatment.

At this visit we will gather records to evaluate your oral health. With this information we will be able to answer your questions and educate you about your dental health. We will be able to provide you with treatment options and the time and financial commitments of each so that you are able to make the best decision for your dental health. Please set aside 90 minutes for this visit.
